If I buy a roaming pack of, say 8 days, before my trip (North America), and find I will be staying longer than that, can I just buy another 8 days while there? Or must I set it all up before I go?

Purkoy yes you can just purchase another roaming add-on if you end up extending your stay. The relevant part of the terms is below.
If you purchase an EU Roaming Add-On, you can use the allowance for data, making & receiving calls and sending texts locally within your destination country or back to the UK. This add-on will not auto-renew. In case you run out of your allowance, you can buy another add-on, or standard Pay as you go rates will apply. If you do not buy a roaming add-on, you will need Pay As You Go top up credit to use on PAYG basis while in roaming.
