Mcdonald I've removed your mobile number as the community is publicly visible.
A few options for getting your PUK code are here:
If you still have it, look at the back of the card your SIM was attached to when got it and the PUK code should be there.
Log in to MyLebara, go to 'Account', 'Manage my account', and select 'Show my PUK code'.
Customer Support: Contact Lebara directly if the above options are unavailable.
