Lost SIM

Hi
I'm hopeful someone can help.... I'm paying for a plan monthly and yet haven't ever used the sim. It's not showing in my app either but i must've activated it but then realised I that I needed my ex wife to get the PUK code which has never happened. Can I just request a new sim? What is the best course of action?
